Following questions are asked by the interviewer during the safety officer interview regarding Lockout Tagout


What is LOTO or what is the purpose of LOTO?

Plant or equipment are being shut down for repair or maintenance purpose. The equipment contains hazardous energy such as chemical, electrical, hydraulic, mechanical, pneumatic etc. that can cause injuries or fatalities to the worker who engaged in maintenance or repair or line braking activities and nearby people after suddenly release. Without proper follow-up of the LOTO procedure, this energy led to injuries or fatalities.

 

What are the 6 steps of lockout/tag-out?

  • Shut down the equipment
  • Make sure that there is no residual energy within the system that may cause injury
  • Put Lock and Hold Tag   
  • Clear the area and notify affected employee,
  • Make sure by trying to start the equipment to ensure that equipment would not start
  • Lock removal and re-energizing

What does Lockout Tagout mean?

Prevent unexpected start-up of equipment or release of store energy until the maintenance or operation finished their work. The procedure used to make employee safe from unexpected start-up machinery or suddenly release of energy that could be injured or killed them.


Lock-out procedure:

procedures generally include the following steps:

  • Identify the energy source, primary energy (electrical) or stored energy
  • De-energize equipment.
  • Lock-out and Tag-out
  • Verify that the equipment is isolated.


The device which is used to isolate equipment are:

  • One Locks One Key
  • Chains
  • breakers

 

Tag-out procedure

A tag must be installed on each lock, the following information must be clearly recorded on the hold tag:

  • Date and time
  • plant number or location,
  • equipment name,
  • badge number,
  • contractor name,
  • contact phone,
  • signature of the individual who installed the lock, and
  • the reason for the lock. 


What is meaning of Clear?

Clear means to remove all unnecessary personnel and equipment from the area in preparation for a test of the lock-out. This is done to avoid injury to personnel or damage to equipment in the event that the lock-out was unsuccessful and the equipment energizes creating a hazardous situation.


What is the meaning of Try?

Try means to verify the isolation of the equipment by operating all controls on the equipment to make sure the equipment cannot be started until the lock is removed.

 

Lock removal and re-energizing

Authorized personnel must make sure that serviced equipment has been returned to a functional condition. The lock and tag must not be removed until operations personnel have checked and examined the equipment to ensure safe re-energizing.
